Insurance Information

Billing has become increasingly complex due to the proliferation of managed care companies. If we have signed a participating agreement with your insurer, we will bill them. You are responsible for your co-pay at the time of service, and any remaining balance from services rendered. If we do not participate in your insurance plan, payment is expected at the time of your visit. It is ultimately your responsibly to understand your insurance benefits with your insurance carrier. Some policies have very intricate rules about what is and is not covered. Chicago Gastro is always subject to the final determination of coverage by your insurance plan.

Dr. Albert accepts most private insurances, self-pay, and the following plans:

If your plan is not listed, please call the office at 773-368-3164 for more information. Insurance plans are subject to change at any time. Remember to bring a valid insurance card, photo ID, and your co-payment (if applicable) to each appointment.

Please note that we accept cash, checks and credit cards for co-payment or account balance payment.
